ADULT FAMILY CARE HOMES
PURPOSE:
Adult family care homes are a community alternative to other living situations, providing supervision and support in a home setting.
ELIGIBILITY:
Serving all target groups. Must go through a placement process when Sawyer County Health & Human Services is involved. Rate is dependent on care needs. Several programs will fund these alternative living situations. Several facilities are licensed in Sawyer County. Size of facilities vary from one person to four residents.

COMMUNITY BASED RESIDENTIAL FACILITIES (CBRF)
PURPOSE:
To provide assisted living as an alternative to nursing home care for adults in need of supportive services. Staff available 24 hours a day; three meals a day.
